What is the best pain relief during labour?
Q. Ive never been good with pain, Ive always had extremely painful period pains in life and in general have never been good with pain. So now that i am pregnant and planning my labour, I would like to know what is the best pain relief as I dont want to have a stressful, painful labour. Is gas and air going to be enough? does it actually take any pain away? Or would an epidural be a better option? What are the pro's and con's for both of these?

A. It depends on your situation at the time and when you end up in hospital in relation to the progress of your labour. If you are in advanced labour gas and air becomes the only option. If you wish an epidural, which is not without risk and increases your risk of requiring intervention, (forceps or a Caesarean,) ideally you should discuss this option with your midwife in advance. Otherwise the middle option is pethidine and gas and air in the later stages.

How much pain should you expect from a stress fracture in your femur?
Q. I am 12 and have had a stress fracture for about 4 months, I am on crutches and I'm starting physical therapy on Tuesday. I am in EXTREME pain during the night, and moderate pain during the day. How long will this go on? Help, please and thank you.

What are the laws regarding prescription pain meds(including methadone)when driving class A trucks?
Q. I am planning on getting my class A license and a long haul truck driving job. I have had to take pain meds in the past for back pain. Is it legal to take prescription pain meds while driving the big rigs. The pain meds that work for me are oxycodone(percacet) or methadone.

A. I know that it is against the law to use methadone and drive a truck but I am not sure about the other meds. I would call Human Resources and ask what the policy is regarding prescription medications and driving.

Pain is the unpleasant and aversive feeling common to such experiences as stubbing a toe, burning a finger, putting iodine on a cut and bumping the "funny bone". The International Association for the Study of Pain defines pain as "an unpleasant sensory and emotional experience associated with actual or potential tissue damage, or described in terms of such damage".

Pain motivates us to withdraw from damaging or potentially damaging situations, protect the damaged body part while it heals, and avoid those situations in the future. It is initiated by stimulation of nociceptors in the peripheral nervous system, or by damage to or malfunction of the peripheral or central nervous systems. Most pain resolves promptly once the painful stimulus is removed and the body has healed, but sometimes pain persists despite removal of the stimulus and apparent healing of the body; and sometimes pain arises in the absence of any detectable stimulus, damage or pathology. Cultural values, hypnotic suggestion, and cognitive activities such as distraction or appraisal can all significantly modulate pain's intensity and unpleasantness.

Pain is the single most frequent reason for physician consultation in the United States. It is a major symptom in many medical conditions, and can significantly interfere with a person's quality of life and general functioning. Pain medicine is a subspecialty under such medical specialties as anesthesiology, physiatry, neurology, and psychiatry. The study of pain has in recent years attracted many different fields including pharmacology, neurobiology, nursing, dentistry, physiotherapy, and psychology.
